The DAC Appui Santé Lille Agglo is the result of the unification of the MAIA Lille Agglo, palliative care network and Lille Agglo geriatric health network.He intervenes in support of professionals on different missions:- information and orientation of professionals towards the health and medico-social resources of the territory,- support for the coordination of the health pathways of people in complex situations to ensure that they stay at home,- territorial animation and observatory of ruptures in care pathways.The territory covered is 38 municipalities of the Lille Metropolis (including Lille, Hellemmes, Lomme, and from Quesnoy sur Deûle to La Bassée)
