The USD-4D app is intended for patients of healthcare providers who participate in the USD-4D project of Expertise Center Palliative Care UMC Utrecht.The Utrecht Symptom Diary helps patients to gain insight into their own symptoms and concerns and is used by the healthcare provider to talk to them about these symptoms.
By regularly filling in complaints and/or problems that you experience as a result of your illness, you and your healthcare provider can together determine, evaluate and, where necessary, adjust the most suitable care for you.
The questions can also be answered by a loved one or a healthcare provider if you are unable or unwilling to do so yourself.The Utrecht Symptom Diary is a diary used by your healthcare providers to talk to you about what is bothering you and how to improve care.
The diary is not used to continuously monitor you!
The healthcare provider looks at what you have filled in during or shortly before the interview with you.
