Fluid Heart Tracker is a self care tool, designed as a digital log of daily weight to promote independence and empower the person to recognise an increase of 2 kg in weight in a 7-day loop period and seek medical advice.
It allows heart failure patients to capture their weight every day.It was developed with heart failure community nurses in the HSE who were concerned that their patients would be afraid to go to their GP/ services due to lockdowns.It is not a substitute for medical observation.
If you are breathless or have chest pain, you must seek medical advice immediately.If the weight logged has increased by two kilograms or more in seven days, an alert is displayed until the user acknowledges the alert.
