This app is based on the second edition of the Scottish Governments national guidance on polypharmacy. This builds upon and refines the previous guidance from 2012. The โ€˜7-stepsโ€™, defined within this guidance, aim to provide a clear structure for the medicines review process, centred on the needs of the individual as a whole, and encouraging the dialogue between clinician and patients to include non-pharmacological solutions as well as medication ones. This guidance aims to support those carrying out comprehensive face-to-face medication reviews with patients and where appropriate carers/welfare proxies. It also contains much information that patients will find useful.The app has been developed jointly by Scottish Government, NHS Education for Scotland, two subgroups of the Polypharmacy Guidance Review Group - the Model of Care Group and the Data, Indicators and Evaluation Group and input from the Medicines Information pharmacist network.Updated splash screen and name change

Attractively presented but you'd go mad trying to do an actual polypharmacy review trying to read through all these screens...just putting all the existing guidance into a smartphone 'toolkit' is not really an app...

Doesn't work

Can't get the app to open on my new Samsung S8

Loved this app when it worked on my previous phone (HTC). Doesn't seem to open on my One Plus 5T which is a real shame! Please fix!

When I first got this app it wasn't that useful. Following the full guidelines for a polypharmacy review takes time and the app didn't really streamline that. However now I find this app helpful in practice on the wards and in clinic. The NNT and cumulative ADR pages are really informative when weighing up discontinuing a medicine and the Hot Topics cover some tricky areas. The rest is a handy reference to the wider polypharmacy guidance.
