It manages your signed consent forms and medical questionnaires, appointment bookings and client picture uploads with storage, while protecting your business from malpractice liability.

Clients Records and Forms App provides consent form creation from templates, authoring of medical questions for patient history, therapy image uploads, appointment bookings, the sending of online forms to clients for completion with signatures and a business portal where patients can sign up, make bookings and communicate with your practice.
The forms themselves can be sent to patients via email, sms and social messaging. The patients then fill-out and sign the forms online from their phones, tablets or PCs.

In the way of free advertising and marketing, your business details and logo are listed on the portal, while your business will also have its own portal for your clients to book themselves appointments online.
