My Oosterwal Center gives patients access to a selection of the data known to us in a secure and easy way. Also, you can help your own treatment process by filling out questionnaires. The results of these surveys provide insight into disease progression and the effect of the treatment here.

Eczema questionnaire (POEM)
The Patient Oriented Eczema Measure is considered the best survey to measure the severity of eczema such as the patient experiences it.

Quality of life
These four questions are derived from the Skindex-29 and provide an insight into the development of the overall quality of the patient during the course of the treatment.

Itching / Crab Diary
With the itch / crab diary be able to recognize the patient and physician detailed patterns in the severity of itching and the tendency to scratching.

Research
The above questionnaires are part of scientific research in collaboration with an aside and Skin Patient Netherlands. Analyses will be performed solely anonymously and with the consent of the patient.

Other surveys
To promote the process of care questionnaires can also be offered to you. These include questionnaires regarding your medical history as well as questionnaires related to the service.

Your medical records
In your medical file shows only medical information that is added to your file after January 1, 2017. Data from before this date will have to see your doctor and healthcare team. The following parts can be seen in your file:
